What Is the National Health Insurance Program?

National Health Insurance is a government program run by BPJS Kesehatan to provide health protection for all Indonesian citizens. The program is governed by Law No. 40 of 2004 on the National Social Security System and Law No. 24 of 2011 on BPJS.

According to Article 1 paragraph (1) of Law No. 40 of 2004, "The National Social Security System is the procedure for administering social security programs by several social security administering bodies." This statement makes clear that JKN is not just a health service but part of a structured social protection system.

The program has run nationwide since January 1, 2014, and it keeps developing in its services, technology and membership coverage.

Goals of the JKN Program

The main goal of JKN is to guarantee that every member receives the benefit of health care and protection in meeting basic health needs.

BPJS Kesehatan itself states that the program is designed to make sure no one falls into poverty because of medical costs. This principle is known as financial protection, an important foundation of any modern health system.

JKN also aims to:

  • Improve access to health care evenly
  • Improve the quality of medical services
  • Encourage efficiency in national health financing
  • Reduce gaps in services between regions

How the JKN Program Works

JKN is built on the principle of gotong royong. This means healthy members help members who are sick by paying regular contributions.

In outline, the program works as follows:

1. Membership and Contributions

Every citizen is required to be a JKN member. Members are divided into several categories, such as:

  • Wage earners (Pekerja Penerima Upah, PPU)
  • Non-wage earners (Pekerja Bukan Penerima Upah, PBPU)
  • Contribution assistance recipients (Penerima Bantuan Iuran, PBI), whose contributions are paid by the government

Contributions are paid every month according to the class a member chooses or according to government rules for certain members.

2. Tiered Access to Health Care

Members do not go straight to a large hospital. They must first go through a primary health facility (FKTP), such as a community health center (puskesmas) or a clinic.

If needed, the patient is referred to:

  • An advanced-level hospital
  • A specialist doctor
  • Other advanced health facilities

The purpose of this referral system is to keep services efficient and evenly distributed.

3. Payment by BPJS

After a member receives care, BPJS Kesehatan pays the costs according to the applicable rules. Payment to health facilities uses a capitation method (for FKTP) and INA-CBGs (for hospitals).

According to Presidential Regulation No. 82 of 2018 on Health Insurance, this system is designed to control costs while maintaining service quality.

Benefits of the JKN Program

JKN provides many significant benefits, both for individuals and for society at large.

Protection from Health Costs

Treatment costs, especially for serious illnesses, are often very high. JKN helps cover these costs so members do not have to pay large sums out of pocket.

Wider Access to Services

Members can access a wide range of services, from basic checkups to complex medical procedures such as surgery and inpatient care.

More Even Health Services

With the referral system and partnerships with many health facilities, services reach more evenly into the regions.

Prevention and Promotion

JKN does not focus only on treatment. It also covers promotive and preventive services such as immunization and routine health checkups.

Services Covered by JKN

In general, the services covered include:

  • Doctor examinations and consultations
  • Outpatient and inpatient care
  • Childbirth
  • Medical procedures and surgery
  • Medicines listed in the national formulary

However, some services are not covered, such as:

  • Cosmetic care
  • Alternative treatments that have not been medically proven
  • Conditions resulting from criminal acts or intentional acts

These rules are set out clearly in BPJS Kesehatan regulations.

Challenges in Implementing JKN

Despite its many benefits, the program also faces a number of challenges.

Financial Deficit

One of the main issues is the imbalance between contributions and service claim costs. This often creates a deficit in managing the fund.

Service Quality

There are still complaints about long queues, limited facilities and the quality of service in some regions.

Contribution Compliance

Many self-paying members do not pay their contributions regularly, which affects the sustainability of the program.

Distribution of Health Facilities

The gap in facilities between big cities and remote areas is still a problem that has not been fully solved.

The Role of BPJS Kesehatan as Administrator

BPJS Kesehatan plays a central role in running the program. Its main tasks include:

  • Managing members' contribution funds
  • Working with health facilities
  • Guaranteeing payment for health services
  • Developing digital systems and services

According to Law No. 24 of 2011, Article 5, "BPJS aims to realize the provision of guarantees that the basic needs of a decent life are met for every member." This makes clear that BPJS is not just an administrative body but the main pillar of the national social security system.

Digital Transformation in JKN

In recent years, BPJS Kesehatan has begun adopting digital technology to improve its services. Examples include:

  • The Mobile JKN app for registration and online queuing
  • A digital referral system
  • Data integration with hospitals and clinics

These steps aim to improve efficiency, transparency and ease of access for members.

Conclusion

The National Health Insurance program is an important foundation of Indonesia's social protection system. Built on the principle of gotong royong, it gives people wider access to health care while protecting them from the financial risk of medical costs. Although it still faces many challenges, JKN remains a strategic step toward an inclusive and sustainable health system.
